Light-controlled therapeutics
Does light become a paid therapeutic control layer beyond dermatology once the first optogenetic, photoimmunotherapy and endoscopic light-drug approvals arrive (2027-2029)?

The call, a draftThe approvals side is ahead: an optogenetic eye therapy, a bladder drug lit through a scope and a near-infrared tumour treatment are all near a decision. Light still only reaches about a centimetre into tissue, so the window is early.
- D1Nanoscope's optogenetic gene therapy MOGENRY had its FDA licence application accepted in Sep 2026, with a decision expected in 2027.
- D2Theralase plans to file Ruvidar for BCG-unresponsive bladder cancer from 3Q 2026, with light delivered through a cystoscope in a fully enrolled 90-patient trial.
- D3Rakuten Medical's near-infrared photoimmunotherapy is already approved in Japan as Akalux, with a US filing targeted for 2028.
- D4The 2026 Nobel prize for optogenetics and GenSight's NEJM paper of 7 Oct 2026 put the field in front of buyers and acquirers.
- R1Useful light reaches about 1mm into tissue at blue wavelengths and about 1cm at red, so the brain and heart need an implant or a converter particle that nobody has shown in a mammal.
- R2Red and near-infrared photoswitches have only worked in tadpoles and brain slices so far, with no mammal result.
- R3GenSight has about €0.7M of cash and a going-concern warning, and RetroSense's asset failed in Phase 1 under AbbVie.

- 12 Oct 2026Light-controlled therapy is two or three years from its first US approvals outside the skin, and all of them are in places light already reaches: the eye, the bladder through a scope, a tumour lit by a laser.
- 12 Oct 2026The money in light-controlled therapy is betting on geographic atrophy and an IPO window after the first optogenetic approval in 2027, not on light as a category.
